Top-Rated Longevity Compounds

Ranked by evidence quality, mechanism clarity, safety profile, and practical accessibility.

1

Omega-3 (EPA/DHA)

Decades of RCT data. Clear mechanisms (inflammation, membrane fluidity, gene expression). Dose-dependent benefits confirmed in REDUCE-IT and VITAL trials.

96
2

Vitamin D3

Regulates 1,000+ genes. Deficiency linked to virtually every age-related disease. Supplementation is safe, cheap, and measurable via blood test.

95
3

Creatine Monohydrate

One of the most studied compounds in all of sports science. Emerging evidence for neuroprotection, sarcopenia prevention, and mitochondrial support beyond muscle.

94
4

Magnesium (Glycinate/Threonate)

Cofactor in 300+ enzymatic reactions. Over 50% of adults are deficient. Glycinate for general use; threonate for cognitive support.

93
5

NMN (Nicotinamide Mononucleotide)

Strong mechanistic basis for NAD+ restoration. Robust preclinical data. Human trials showing metabolic and vascular benefits. Purity varies wildly by brand.

88
6

CoQ10 (Ubiquinol)

Essential for mitochondrial ATP production. Natural levels decline with age. Ubiquinol form is pre-reduced for better absorption. Mandatory if on statins.

86
7

Fisetin (Senolytic Protocol)

Mayo Clinic identified as most potent natural senolytic. Acute dosing protocol (not daily) has strong preclinical support. Human clinical trials underway.

82
8

Spermidine

Autophagy inducer with epidemiological support (Bruneck study). The only compound associated with reduced all-cause mortality in observational data from dietary intake.

79
9

Urolithin A

Mitophagy activator with published human RCT data (Mitopure). Addresses mitochondrial dysfunction directly. Limited by being relatively new with one primary commercial source.

76
10

Alpha-Ketoglutarate (CaAKG)

Krebs cycle intermediate with Buck Institute healthspan data. TruAge study showed biological age reduction. Still early-stage for human longevity evidence.

How We Score

The Biohack Index Formula

Every compound is scored across five dimensions, weighted by importance to the longevity-focused consumer:

  • Evidence Quality (35%): RCTs > cohort studies > preclinical > mechanistic. Human data weighs 3x more than animal data. Replication matters — one study is a signal, not proof.
  • Mechanism Clarity (20%): Is the biological pathway understood? Can we explain why this works at the molecular level? Compounds with clear mechanisms score higher because they're less likely to be statistical noise.
  • Safety Profile (20%): Long-term safety data, known drug interactions, contraindications, and the therapeutic window (margin between effective dose and toxic dose).
  • Practical Accessibility (15%): Cost per effective dose, availability, storage requirements, and ease of compliance. A perfect compound that costs $1,000/month or degrades at room temperature scores lower.
  • Measurability (10%): Can you verify the supplement is working through a biomarker? Compounds where you can test your response (e.g., vitamin D → blood test) score higher than those where you're guessing.

What We Don't Score

We don't rank branded products in the index — that's what our Stack Reviews section is for. The index scores compounds and protocols, not specific products. We also don't score prescription medications (rapamycin, metformin) — those require medical supervision and are outside the scope of self-directed supplementation.

Updates

Rankings are recalculated monthly as new research publishes. Major scoring changes are flagged in the Biohack News section with an explanation of what changed and why.
